What is your old/current guild, and why are you leaving?:
My old guild was Exploding LabRats. I was an officer here from the beginning to the end. As obviously your all aware, summer has put increasing pressure on raiding guilds. Unfortunately it crippled us beyond repair. I'd like to continue to raid and meet some new people.

If asked to sit out; due to needing a raid slot in a raid, are you one to volunteer or someone who doesn't want to sit out? (Explain yourself):
If for example I attended a raid before which someone hadnt, I would happily offer them my spot. That being said, I do love to raid and am always available on standby if needed.

Im using a brand new computer atm so I dont have any of my screenshots. So i've just highlighted what it would look like in raids. My UI is fairly basic aesthetically but I have all the addons to do my job right. If theres any additional things I need im more than happy to get them.

Please provide the list of consumables you are ready to use each raid, including outdoor buffs(You can do it instance dependent)
Supreme Power, Brilliant Wizard Oil, Shadow Power Elixirs, Greater Arcane Elixirs, Greater Intellect, Food buffs (Rumsey, Kickin Chops, Wolf Ribs etc.), Scrolls, Nature,Fire etc protection potions. I can farm anything I need for a raid.

Please give us a list of the addons you are currently using, to help you in a raid environment, And Do you use any Bigs Wigs analog addon to track boss abilities CD, phase change, etc?:

first of all u have written an interesting apply +1, from what i've read u got experience and gear too (only thing i wonder are the t2.5 boots, why use them without 5/5 - maybe for some stamina fights).
As nuble and holy wondered, " Im also attuned to all raids. " would answer their questions xD. Last thing is, that you will have to say bye bye to your sm/ruin specc after the 16 slot debuff system is out - then roll best specc!
